To reach our new CRC screen for your entire chain (not just a single location) log into your web portal and look at the bottom left for the Manage Chain tab.
Once you’re on the chain management screen click Colon Cancer option on the left, and you’ll see a list of your patients. Use the dropdown menu at the upper right of the screen to filter your patients by the following options.
All patient information on this example screen is made up for demo purposes.
All Patients - This is a list of all your patients who meet the minimum requirement of being at least 45 years old.
Incomplete Screenings - These patients started the screening process, either on their own or with someone on your team and did not finish it. They also did not give an indication that they are no longer interested.
Invited to Self-Screen - These patients have been sent an invitation to complete a self-screen but did not start that process. You will see the date that they received the invite in the Status column.
Interested Leads - A patient has previously indicated interest in being screened for colon cancer but did not
Ordered but Not Resulted - These patients completed the screening, were deemed eligible and completed an order for an at-home test kit. They have not yet received results, which usually means they did not complete the test or mail it back. See below for more info.
Referred for Transient Conditions - These patients completed the screening and are not eligible for an at-home test kit. They have been told that they should consult with their doctor for better screening options. After an appropriate amount of time this status will change to "May be eligible if temporary condition resolved."
Screened & Eligible - These patients have completed the screening, either on their own or with someone on your team. They were deemed eligible for an at-home kit, but did not place an order for one. See below for more info.
Opportunities
If you are looking for patients to contact to encourage them to complete the at-home test and close that care gap, the most effective groups to target are “Ordered but Not Resulted” and “Screened & Eligible.”
Patients in the Ordered but Not Resulted group have a kit at home, so a call or message encouraging them to complete the test and send it in can go a long way.
Patients in the Screened & Eligible group know they can receive a kit but haven’t ordered. A message to remind them that the test is usually covered by insurance and is an important way to stay on top of their health can be a very helpful nudge.
Patients in the Referred for Transient Conditions group were interested in the past but couldn't do the at home screening for a reason that may have resolved itself by now. This is a good secondary group to reach out to to either help them order or close them out for good.
