POLICY
The pharmacy may provide Medi-Cal MTM services through telehealth when appropriate and when all Medi-Cal and California telehealth requirements are met.
PROCEDURE
1. Determine if Telehealth is Appropriate
- Confirm the patient is eligible for MTM.
- Confirm the pharmacist is qualified to provide MTM.
- Determine that the MTM service can be completed through telehealth.
- Obtain patient consent to utilize telehealth to deliver MTM.
2. Determine the Telehealth Method
- Offer video telehealth when required.
- If the patient meets Medi-Cal requirements for audio-only telehealth, the pharmacist may complete the MTM service by telephone.
- Document the patient's audio-only request or eligibility when required.
3. Complete the MTM Encounter
- Use Doximity for encounters.
- Confirm the patient's identity.
- Conduct the call in a private setting.
- Complete the MTM service and provide appropriate counseling and recommendations.
- If the service cannot be safely or appropriately completed by audio-only telehealth, arrange an appropriate alternative.
4. Document the Encounter
The pharmacist must document the MTM service in the patient's medical record, including:
- Date of service
- MTM assessment and services provided
- Counseling and recommendations
