California Medi-Cal MTM Pharmacist Qualification Requirements

Overview

To provide Medication Therapy Management (MTM) services under the California Medi-Cal MTM program, pharmacists must meet the qualification requirements established by Medi-Cal before a contract can be approved.

Documentation of a qualifying credential must be submitted to Medi-Cal as part of the contracting process.

Eligible Pharmacist Qualifications

A pharmacist providing MTM services must possess at least one of the following qualifications:

  • Advanced Practice Pharmacist (APh) licensure
  • Board Certification in Medication Therapy Management (NBMTM)
  • Board Certified Pharmacotherapy Specialist (BCPS)
  • A similar Medication Therapy Management (MTM) certification

Examples of MTM certification programs include:

  • American Pharmacists Association (APhA) Delivering Medication Therapy Management Services Certificate Program
  • California Pharmacists Association (CPhA) Delivering Medication Therapy Management Services Certificate Program
  • PowerPak Medication Therapy Management Certificate Program

Note: If you have completed an MTM certification program not listed above and are unsure whether it meets Medi-Cal's requirements, please contact Medi-Cal directly for guidance before submitting your contracting paperwork.

Pharmacist Interns

A pharmacist intern may provide MTM services only under the direct supervision of an eligible pharmacist who meets one of the qualification requirements listed above.

Required Documentation

Before your contract can be submitted to Medi-Cal, you must provide documentation of your qualifying credential.

Acceptable documentation includes:

  • MTM certificate of completion
  • Advanced Practice Pharmacist (APh) license
  • NBMTM certification
  • BCPS certification
  • Documentation of another qualifying MTM certification

Documentation should clearly include:

  • Pharmacist name
  • Name of the credential or certification
  • Completion or issue date
  • Expiration date (if applicable)
