How to Update Your Taxonomy Code for Clinical Pharmacy
If you need to update or verify the taxonomy code associated with your pharmacy's NPI number, follow these steps:
1. Log into the NPPES Portal
Visit the National Plan and Provider Enumeration System (NPPES) website at https://nppes.cms.hhs.gov and log into your account using your NPI credentials.
2. Select "Manage NPI"
After logging in, navigate to the "Manage NPI" section and select the NPI number associated with your pharmacy. OR
Select the pencil icon in the "Action" column of the NPI you wish to modify.
3. Update or Add a Taxonomy Code
Under the "Taxonomy" section, review the current codes listed. If the correct code is not there or needs to be updated, click the "Add" or "Edit" button to make changes. Add the following code:
261QP2300X (Clinic/Center Primary Care)
261QH0100X (Clinic/Health Center - Health Services)
It is important that your new Medicaid ID's taxonomy code match the primary taxonomy code associated with your new Type 2 NPI.
